The ECHO aims to increase access to care for American Indian and Alaska Native people by providing skills of health care provision\, disease control\, and prevention and help to eliminate health disparities by supporting the workforce to improve Social Determinants of Health (SDOH) using a community-based approach. The program will offer a free ECHO collaborative to continue learning\, knowledge sharing and support during virtual ECHO clinics focused on relationship and trust-building\, communication and social determinants of health. 2024 topics include: \n\nThe CHR Strategic Plan (February 26th) – Michelle Archuleta\nCHR/CHW Competency Overview: Tracking and Tools (March 18th) – Dr. Samantha Saba and Louisa O’Meara\nCHR E-learning Basic and Advanced (April 15th) – Whitney Moseley\n‘See It\, Stop It\, Prevent It’ Elder Abuse (May 20th) – Bethany Reed\nCHWs and CHRs in South Dakota – Accessing Medicaid Reimbursement to Sustain the Workforce (June 17th) – Ben Tiensvold\, Vicki Palmreuter\, Catherine Jeffries\nEmergency Preparedness Training (July 15th) – Janet Yellowhair and Adrianne James\nCHR Home Visiting (August 19th) – Pamela Aguilar\nProgram SOW and Position Descriptions: Tools and suggestions to communicate to partners (September 16th) – Bo Flores\nCHR Mini-Cog Screening (October 21st) – Michelle Archuleta and Dr. Jolie Crowder\nCHR Elder Care (November 18th) – Lanette Walker\n\nThe 1-hour telehealth sessions will offer community-based health care workes an opportunity to engage with peers and an interprofessional team of specialists working in Indian Country\, and an opportunity to obtain a Certificate of Completion following participation in a didactic presentation. The sessions take place virtually using the Zoom platform and are held the 3rd Mondays of every month at 12pm PT starting in February 2024.
